69 year old male with uremic pruritis

Chief complaints -
~itching all over the body since 20 days
~ burning micturition
History of present illness
Patient was apparently asymptomatic 20 days back then he developed itching all over the body
There was decreased urine outflow and burning micturition since 15 days
No history of fever
Past history
There was history of swelling and necrosis of right leg 1 year back for which the pt. Has undergone a surgery
There is no h/o HTN,DM, TB, epilepsy , thyroid abnormalities
